Title: Patient Care Coordinator Revised: June 2022
Reports To: LCSW
FLSA: Non-Exempt

Position Summary:
Under the supervision of an LCSW, the Patient Care Coordinator is responsible for coordinating with clinical staff to support patients to receive comprehensive care and referrals essential to health and wellness. Also, needs to be responsible for case management and consultation with current case managers of specialty and linkages to community-based partners and resources.
Must be able to interact positively and assertively with current patients and the public, and have excellent de-escalation skills for managing crisis situations within the clinic. Will be the liaison for community outreach and have knowledge of all SDFHC opportunities to facilitate access to health, disability and public assistance benefits programs for all patients. The Patient Care
Coordinator also works with service providers, to engage resources and achieve coordination of care for the patient and maintains all necessary documentation.

Qualifications:
Bachelor’s Degree in Social Work highly preferred. Bilingual in English/Spanish required.
Experience in social services. Strong interpersonal skills, including the ability to de-escalate patients in crisis and command authority in difficult situations. Must demonstrate team building and authenticity in supervision with the ability to work independently. Knowledge of community resources available to patients, as well as internal referrals. Excellent verbal/written communication and organizational skills. Strong problem-solving skills; adept at multitasking.
Sensitivity and ability to serve clients with diverse backgrounds. Working computer skills including MS Office, EMR and other software systems.

Experience:
A minimum of two years documented experience, within the immediate past five years, in a mental health, medical or social service setting preferred. General knowledge of referral process and follow-up recommendations. Demonstrate community outreach, access to resources, referral submission, networking with diverse professionals and patients from various backgrounds.
Training in de-escalation methods preferred. Must have experience problem-solving and working with challenging populations and be able to maintain an assertive presence with patients and staff.

Additional Requirements:
Ability to pass an initial and annual physical assessment and TB test and pass an initial background check conducted by the agency. Must be able to present a positive image of the organization and establish a warm, friendly and welcoming environment to all visitors to the health centers. Must have effective communication skills, both oral and written, and strong organizational, problem solving, time management, customer service and multi-tasking skills.
Attention to detail, quality focus and customer service skills are essential. Must be knowledgeable of office and medical equipment/machines and Practice Management/Electronic
Medical Record systems. Ability to carry out the duties and responsibilities of the job with

minimal supervision and make decisions where the decision making involves good judgment.
Must be able to apply and comply with agency policies and procedures as well as all regulatory standards.
Expected to obtain within 90 days of hiring a CPR and First Aid Certification and all training required for MHLA, MediCal, and Covered California enrollment.

Working Conditions:
Due to the nature of Community Health Centers, this job involves exposure to blood, bodily fluids or tissues and possible exposure to communicable diseases. This job involves standing for long periods of time as well as sitting, stooping or squatting and must be able to lift 50 pounds.

Schedule:
Generally Tuesday – Friday 8am- 5:30, Saturday 8a-430p. However, the schedule may change depending on the needs of the agency.

Essential Functions (includes but not limited to):

1) Assists with benefits for non-disabled and disabled individuals (i.e., families, General
Relief, food stamps, etc.);

2) Assists during local, State and Federal audits pertaining to program eligibility;
3) Participates in SDFHC outreach functions as requested or required;
4) Participates in all required management and staff meetings including weekly staff meetings, special task forces as may be required;
5) Helps patients in distress by responding to emergencies;
6) Provides de-escalation techniques in crisis situations;
7) Protects patients' rights by maintaining confidentiality of personal and financial information, compliance with all HIPAA requirements;
8) Understanding the variety of insurance and health, SUD and mental health maintenance options as well as county-funded and treatment programs throughout LA County available and the importance of helping patients access those benefits;

9) Understanding diverse cultures and incorporating the relevant needs of culturally diverse groups, as well as people with disabilities;

10) Establish and maintain relations with civic groups, agencies, other professionals, governmental entities, and the community at large to ensure appropriate referrals, identify service gaps, expand community resources, and help to address unmet needs;

11) Differentiate between situations in which it is more appropriate for the patient to self-refer to a resource and those in which counselor referral is required;

12) Arrange referrals to other professionals, agencies, community programs, or other appropriate resources to meet patient needs;

13) Exchange relevant information with the agency or professional to whom the referral is being made in a manner consistent with confidentiality and compliance with all HIPAA requirements;

14) Evaluate the outcome of the referral and initiate collaboration with the patient and the referral source;

15) Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations;

16) Contributes to team effort by accomplishing related results as needed;
17) Runs eligibility for patient BH sessions daily;
18) Maintains organizational skills for collecting client payments;
19) Provides enrollment services for Newhall health center;
20) Performs other duties, as requested.

Job Type: Full-time

Pay: $19.00 - $21.00 per hour
