Our organization is looking for an experienced Referral Coordinator to join our team. The successful candidate will be responsible for coordinating referrals between healthcare providers and other organizations, ensuring that all paperwork is accurate and up-to-date. The Referral Coordinator will also be responsible for providing customer service to clients, communicating with healthcare providers, and maintaining accurate records. We are looking for an individual who is organized, detail-oriented, and has excellent communication skills.

Responsibilities:

• Monitor and track referral requests.
• Maintain accurate records of referrals and follow-up activities.
• Respond to inquiries from patients, families, and healthcare providers in a timely manner.
• Coordinate referrals with other healthcare providers and agencies.
• Provide education to patients and families about the referral process.
• Assist in the development of referral protocols and processes.
• Assist in the development of referral policies and procedures.

Qualifications:

• preferred experience as referral coordinator
• Bilingual in Spanish is a plus

Job Type: Full-time

Pay: $15.50 - $17.00 per hour
