Job Description:
Summary of Responsibility
Provides comprehensive assessments, group and individual counseling, community support, case management; serve as advocate to families; provide information, education & support; promote wellness, recovery & skill building to sustain better health; provide training to manage stress/problems and make healthy lifestyle choices; recognize & respond to relapse, manage crisis & achieve and sustain recovery. Assists clients with personal recovery goals, helps clients develop a support system within the family, and encourages clients to advocate for themselves within their healthcare systems. Implements evidenced based parenting programs. Flourish Grant Counselor works both autonomously and under the direction of assigned supervisory staff when performing job tasks/providing services. As part of Centerstone’s team-oriented philosophy, a Flourish Grant Counselor is expected to actively participate in variety of cross-functional teams and performance improvement initiatives.
ESSENTIAL DUTIES
Completes job requirements with independent action and resourcefulness and seeks out additional duties during periods of downtime. Organizes time, effort, and use of resources to achieve desired outcomes.

Treatment must be recovery focused taking into account trauma informed care practices. Provides consistent, personal growth-focused services to all assigned clients by maximizing time spent working directly with clients to assist their personal growth process. To demonstrate clients are the first priority, ensure hours of care expectations are met and time is dedicated to providing personal growth services to clients, their families and collateral contacts.

Provides outpatient services utilizing evidence-based methodologies to provide excellent clinical services. Prepare for and conduct individual, group, and family services for assigned caseload.

Completes all required documentation legibly, accurately, timely, and thoroughly using clinically appropriate terminology and containing clinically relevant information and applicable standards. Demonstrates utilization and incorporation of audit feedback when completing documentation.
Supports the strategic goals and mission/vision/values of Centerstone at all times when interacting with clients, co-workers and professionals from other organizations and presents a positive and professional demeanor.
Takes initiative to prevent and resolve problems and offers recommendations for improving services and transforming Centerstone into an organization that embraces excellent customer experience.
Assist clients with making linkage with internal and external resources as outlined in service plan and in coordination with integrated healthcare services. Maintain appropriate contact with referral sources including providing information on client progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
Assist client in making discharge plans and provide follow up services as assigned.
Demonstrates an ability to manage crisis situations, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated.
Attend staff meetings, clinical staffings, and other internal meetings as assigned. Participate on committees as assigned, including quality assurance functions. Maintain appropriate communication with other employees and work as an effective member of treatment team.
Immediately begins the engagement process with newly assigned clients and their families by documenting contact within 3 business days of the referral.
Demonstrates commitment to all assigned clients and families at various stages of change by using motivational interviewing and outreaching clients who have disengaged from services. Provides assertive outreach, advocacy and follow-up to assigned clients. For disengaged clients and families, a minimum of 3 attempted phone outreaches and 1 letter of interest (as consents allow) are completed and documented in the Electronic Health Record.
Throughout the personal growth process, ensures services provided are driven by client personal growth goals. Links all clinical documentation to identified personal growth goals and objectives as indicated on the individual service plan. Ensures documentation of progress note activities reflect personal growth plan objectives and demonstrate medical necessity. Utilizes progress note completion as a personal growth tool to celebrate client successes and identify growth areas by completing progress notes concurrently with clients.
Demonstrates effective caseload management by expeditiously staffing and transitioning client cases based on client needs and preferences. Completes and submits transition plans and discharge summaries.
Completes and submits all required clinical documentation within established timeframes (generally within 1 business day of service provision). To ensure a complete clinical record, completes Progress Notes for all contacts, canceled/failed appointments and attempted contacts with clients and collaterals within established timeframes.
Maintains confidentiality of clients as well as any and all information considered sensitive to Centerstone operations.
Follows HR Policy and Procedure for management of Time Off and/or any other leave. Minimizes unscheduled absences within established Centerstone guidelines (expectation of 2-4 total unscheduled absences per year).
Meet and maintain applicable certification and job requirements.
Trains and/or assists with training staff on evidence-based interventions for substance use as assigned.
OTHER DUTIES AND RESPONSIBILITIES
Market services to external resources as appropriate. Participate in training designed to enhance job-related skills. Provide community education as assigned. Perform other duties, not inconsistent with qualifications and regular duties, as assigned.
ESSENTIAL KNOWLEDGE AND SKILLS

RESPONSIBILITY
Supervisory Controls: The Project Director assigns work priorities, provides ongoing clinical supervision, and interprets and enforces policies and procedures.
Guidelines: Board, Administrative and program policies, licensure, accreditation, statutory and regulatory standards restrict the Flourish Grant Counselor discretion on treatment assessment, services, and record-keeping. A Flourish Grant Counselor has the prerogative to propose changes in practice, procedures and policies that affect the Clinical team.
DIFFICULTY
Complexity: Position is characterized by a series of complex tasks. Although routine is established setting certain tasks at certain times, the extreme variation in service recipient needs requires adaptation. Emergency situations occasionally disrupt routine operations and may require more complex responses.
B. Scope & Effect: This job affects the physical, emotional, and psychological well-being of clients. The agency depends on the Flourish Grant Counselor to appropriately provide clinical services which meet requirements set forth in external grants/contracts and meet requirements for third party reimbursement.
PERSONAL RELATIONSHIPS
A. Personal Contacts: The Flourish Grant Counselor has daily contact with clients, collaterals, and agency staff; regular contact with external referral sources, other service providers and advocates, and general public.
Purpose of Contacts: Purpose of contact with clients is to provide clinical services, assessment, non-violent crisis intervention, and skill building services as needed, as well as consultation regarding these services. The purpose of contact with others is primarily to gather and exchange information.
ENVIRONMENTAL DEMANDS
Physical Requirements: The work is primarily sedentary, but requires skill in non-violent crisis intervention. Must be able to lift up to 25 lbs.
B. Work Environment: Work is primarily performed in an office environment or community setting such as a hospital, police department, county jail, school, service recipient home, etc. There is risk due to location of services and/or potential violence of service recipients in crisis. Position may require contact with bodily fluids to assess drug use; this requires the use of protective clothing.
MINIMUM CREDENTIALS/REQUIREMENTS
Requires a Master’s Degree in Social Work, Psychology or other human service related field. Experience providing counseling and mental health/substance use services with families is preferred. Masters Level SAS staff must have certification or licensure within the required time frame mandated by SUPR Staff Qualifications (or within 2 years of hire, whichever comes first). Position also requires the ability to utilize necessary computer hardware and software including a working knowledge of personal computers, Microsoft Windows and the Microsoft Office suite. Must have valid driver’s license in applicable programs. Providers utilizing electronic health records and/or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency.
A Master’s level Flourish Grant Counselor must meet Qualified Mental Health Professional status in relevant programs. All employees in the Flourish Grant Counselor position must successfully complete required background checks and pre-employment screening.
