The Community Ambassador will conduct street outreach and one-on-one contact with target population for The Wise Neighbor project, senior citizens residing in the Hill District, Homewood, and East Liberty neighborhoods of Pittsburgh. This role with Macedonia Family and Community Enrichment Center is a professional position working in the development and coordination of aging services, public health education, health promotion, and community outreach activities.

This full-time position supports aging in place and positively influencing social determinants of health. The Community Ambassador will incorporate community issues and concerns into outreach activities with sensitivity, awareness, and skills associated with working with individuals of all ages and backgrounds. A commitment to maintaining excellence in all phases of the project is essential.

WHAT WILL I DO?

• Conduct a minimum of 20 hours of street outreach per week by visiting senior living facilities, residential homes, and other identified locations to engage with older persons over the age of 60.
• Maintain outreach tracking forms and documentation; prepare reports when necessary.
• Conduct informational workshops and facilitate consumer information groups/event.
• Act as a liaison with community organizations and Advisory Council as it relates to supporting senior citizens.
• Provide basic information on health resources, financial budgeting, housing support, food insecurity, or other relevant resources.
• Interview and elicit pertinent information from community members.
• Provides linkage to referrals and information to clients.
• Work closely with Community Engagement Case Manager to connect consumers to the program.
• Coordinate information and/ or services with other social services and healthcare entities to meet the needs of senior citizens.
• Participate in ongoing trainings and remain knowledgeable in areas of expertise (aging services, social determinants of health, healthcare).
• May work evenings and weekends with advance notification.
• Assist in city-wide events.

EXPERIENCE/MINIMUM REQUIREMENTS:

• High School Diploma or equivalent required, some college course in social work, public/business administration, education, health or related field preferred
• 2-3 years’ community engagement experience
• Demonstrated knowledge about the strengths, needs and barriers for senior citizens
• Knowledge of community resources for appropriate referrals
• Demonstrates ability to use motivational interviewing techniques
• Ability to work well with people of diverse social, cultural, and economic backgrounds
• Proficiency with Microsoft Office Suite
• Excellent communication and organizational skills, along with detail-orientation
• Act 33, 34 and FBI clearances needed
• Valid PA driver’s license, clean driving record, insurance and registration required

SALARY: $37,500 to $39,500- commensurate with experience and education background

Because Macedonia FACE serves medically vulnerable consumers, COVID-19 vaccination is strongly recommended.
