FirstLight Home Care - an award-winning non-medical home care agency - is seeking an experienced Geriatric Client Care Manager. The ideal candidate is a highly organized professional; committed to providing quality care to improve the lives of our older adult population. Both full-time and part-time opportunities available; part-time is an ideal way to supplement income.

FULL-TIME POSITION: BASE + BONUS

• NOTE: This position covers one weekend per month; subject to change

Responsibilities of the Client Care Manager:

• Responsible for conducting client intakes / assessments as well as regularly scheduled wellness visits and reassessments after change of condition; of key importance is to enrich lives by finding safe, creative ways to help clients maintain as much independence as possible
• Responsible for developing and maintaining clear and concise client care plans that include physical, emotional, and social well-being; creating extraordinary, thoughtful client experiences every day
• Responsible for communicating and collaborating with clients, families, care partners, and referral sources regularly, ensuring that they are highly satisfied with the care being provided to their loved ones
• Responsible for navigating family dynamics to maximize success in achieving client goal
• Responsible for serving as a mentor to caregivers and supporting their care delivery with a consistent focus on client needs
• Responsible for supporting day-to-day operations with office team to ensure proper client care and coverage / schedules as well as smooth operations and client satisfaction
• Responsible for providing periodic after-hours support as needed by clients
• Responsible for ensuring that FirstLight Home Care continues to be the provider of choice through exceptional care to our clients and by being a true partner to our referral sources; maintaining and enhancing our high standards and best in class reputation

Preferred Qualifications

• Acceptable Candidates: Nursing [LVN or RN], Gerontology, Social Work, Health and Human Services, Occupational Therapy, or other healthcare-related fields
• Minimum Educational Requirements: Bachelors degree in healthcare-related fields
• Minimum Experience Requirements: [1] year in care management working with aging population in home health or home care industry; other professional experience may be acceptable
• Ability to interface effectively and professionally with client supports, including families, physicians, attorneys, etc; ability to build and maintain relationships
• Ability to work in both a collaborative team environment and an independent, self-motivated environment
• Excellent written and verbal communication skills
• Valid driver’s license and proof of current auto insurance

Pay: $65,000.00 - $75,000.00 per year
