Responsibilities/Duties:
- Partner with individuals to develop their recovery goals in relation to their medical, psychiatric and substance abuse disorders.
- Conduct individual and group counseling sessions.
- Assist in program participants aftercare planning and follow-up.
- Caseload management
- Evaluate individuals' progress and report to funders/referral sources as requested through authorization/re-authorization reviews.
- Participate actively in clinical case conferences as a member of the individual's support team.
- Maintain active caseload including engaging individuals in ongoing treatment, maintain tracking system for treatment plans, and other required documents to meet program funder guidelines and program goals.
- Participate in clinical group meetings regarding ongoing treatment goals, progress and discharge as needed.
- Enter documentation into electronic medical records in accordance with agency guidelines.
- Maintain up to date scheduling calendar.
Required Experience:
- A minimum of two years of experience working with individuals with co-occurring disorders.
Required Education:
- Bachelor’s degree in Social Work, Psychology, or related field; Master's in Social Work, Psychology, Addictions, or related field preferred
Required Qualifications:
- A minimum of two years of experience working with individuals with co-occurring disorders.
- Current CADC or LADAC
